Staff Drive: Payroll Deductions

Please read the following carefully before you complete this form:

  1. You may make either a one-time deduction or sign up for monthly deductions. In the box, select the total amount you want to contribute this year. You can contribute an amount other than those present in the boxes by clicking on "Other" and entering the amount.

  2. Once you select the total amount, you must select the number of months from the drop down box next to “Pledge duration”. Do NOT select more than 12 months in the drop down box even though more options may be available. As you change the number of months, it recalculates the amount that will be deducted from your payroll each month. 

  3. If you select the monthly deduction option, the start date list on the form will show a start before November. This is a glitch in the program. Payroll deduction will begin on 11/30/2016.

  4. Under Designation, we have provided a very short list of the many endowment funds and Funds for Excellence (FFX) . If you would like to designate a different fund but want to review some options, click here to download a more extensive list.

  5. In the section marked Billing Address, please enter the address where you would like to receive a thank you letter.
  6. Finally, make sure you click the green "Submit" button at the bottom of the form.
